What you saw. Who you spoke with, if anyone. How the visit ended. Date and time of the attempt Street, unit, and how you identified the building Knocked, waited, spoke with someone, or left as instructed Lights, vehicles, mail, or other observations you actually made Name given, or a short description if they declined a name Served, no answer, wrong address, refused, or access blocked Photos that help Capture the door, unit marker, mailbox, or building identifier at the time of the attempt. Pair the photo with that visit’s note. Do not photograph children, mail contents, or bystanders. Leave these out Legal conclusions, jokes, insults, medical guesses, and arguments with the recipient. Write the observation. Skip the interpretation. How this fits PPS Serve work Assignments are confirmed in writing. Pay is confirmed in writing. PPS Serve does not promise a set number of jobs. If you want to work this way, apply as a process server. Is a photo enough without a written note? No. The note is the record. Photos support the note when they identify the location or the attempt. Should I write that someone evaded service? No. Describe what happened. Counsel decides what the facts mean.
